At Columbia River Home Health, we put people and culture first! We are expanding our team and looking for a compassionate Home Health Registered Nurse Case Manager (RN) in the Tri-Cities area. Never worked in home health? No problem! We offer a ride‑along program so you can see what a career in home health is really like.

• Pay: $45–$60/hour, DOE (PPV equivalent)

• Complete home health intake assessments, medication reconciliation, and care plans
• Collaborate with physicians, caregivers, and interdisciplinary teams
• Educate patients, families, and caregivers on disease management, safety, and recovery
• Maintain accurate clinical documentation and communication with staff

• Current RN license in Washington State
• Minimum 2 years nursing experience; home health experience preferred but not required
• CPR certification
• Licensed driver with reliable, insured vehicle
• Strong communication, clinical judgment, and patient‑focused care skills
