* Manage incoming referrals and coordinate admissions
* Review referrals for clinical appropriateness
* Communicate with patients, families, physicians, hospitals, and referral sources
* Verify insurance benefits and authorizations
* Coordinate seamless transitions of care
* Maintain accurate documentation and patient records
* Build and maintain strong relationships with referral partners### Qualifications
* Current Washington State
** LPN
* * license* Healthcare experience required; home health, hospice, intake, referrals, or care coordination experience preferred
* Strong communication, organization, and customer service skills
* Ability to multitask and work independently in a fast-paced environment
* EMR and computer proficiency
