LPN Licensed Practical Nurse

Life Care Centers seeks a Licensed Practical Nurse to deliver compassionate, patient-first care in our senior living and rehabilitation community. In this role, you'll provide direct nursing care, administer medications, monitor vital signs, support rehabilitation plans, and maintain accurate documentation. You'll collaborate closely with RNs, therapists, and physicians to promote optimal resident outcomes. Responsibilities

• Provide direct nursing care to residents in a senior living and rehabilitation setting
• Administer medications and treatments according to physician orders and facility protocols
• Monitor and document vital signs, patient status, and responses to care
• Assist with development and implementation of individualized care and rehab plans
• Maintain accurate, timely nursing documentation in EHR systems
• Collaborate with RNs, therapists, physicians, and other team members to coordinate care
• Educate residents and families on care plans, medications, and health maintenance
• Uphold infection control, safety, and regulatory standards
• Support residents with daily living activities as needed
• Participate in trainings and quality improvement initiatives
