At Columbia River Home Health , we put people and culture first! We are expanding our team and looking for a compassionate Home Health Registered Nurse Case Manager (RN) in the Tri-Cities area.

• $45-$60/hour , DOE and FTE status (PPV equivalent)

• Complete home health intake assessments , medication reconciliation, and care plans

• Collaborate with physicians, caregivers, and interdisciplinary teams

• Educate patients, families, and caregivers on disease management, safety, and recovery

• Maintain accurate clinical documentation and communication with staff

• Current RN license in Washington State

• Minimum 2 years nursing experience ;
  home health experience preferred but not required

• CPR certification

• Licensed driver with reliable, insured vehicle

• Strong communication, clinical judgment, and patient-focused care skills

• One-on-one patient care in the comfort of their home
