Provide psychosocial assessments to hospice patients and their families Develop and implement individualized care plans Collaborate with interdisciplinary teams to ensure comprehensive patient care Offer emotional support and counseling to patients and families Document patient progress and maintain accurate records Support patients through end-of-life processes with compassion and professionalism What You Need :
Valid state licensure as a Social Worker or relevant certification Experience in hospice or palliative care preferred Strong communication and compassionate bedside manner Ability to work independently and as part of a team Key skills: empathy, organizational skills, cultural sensitivity About the Assignment :
