Functional Eligibility Screener, Full Time, Day Shift, Madison, WI – Sign on Bonus $5,000!
Community Care is hiring a Functional Eligibility Screener who enjoys team collaboration, community-based work, being part of a mission driven organization and who shares our mission to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This position is perfect for someone who is experienced with the LTFS assessment tool and enjoys training and staff support.
This is a full-time opportunity in our Family Care, PACE or Partnership Program based at our Kenosha location. This position has a Sign-On Bonus of $5,000.
Job Responsibilities
- Performs initial, periodic and change of condition functional screens on Family Care, PACE and Partnership Program members.
- Provides ongoing training to all certified staff to ensure staff maintain skills and integrate new information into practice. Develops, provides, and tracks LTCFS training programs for new employees.
- Assists all new Care Management staff in obtaining LTCFS certification.
- Provides intensive oversight of all LTCFS activities for a minimum of 6 months to ensure screener is competent.
- Tracks timely screen completion in computer system.
- Participates in team meetings.
- Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
Job Requirements
- Education
:
Bachelor's Degree in Social Work, Psychology, Human Services or related field is required. - Certification
:
Current Long-term Care Functional Screener. - Experience
: 18 months of experience in a social service capacity or a Master's degree in Social Work, Psychology, or Human Services and one year experience with older adults or persons with physical and or intellectual disabilities in a social service capacity.
- Monday to Friday
- Hours of Operation: 8:00 AM – 4:30 PM
