WhatYou Will Do
As a Licensed Practical Nurse (LPN), you will play a vital role in supporting the health, well-being, and quality of life of our residents by providing compassionate, person-centered care.
Responsibilities Include:- Provide direct resident care services while following individualized care plans.
- Administer medications in accordance with all standards of practice, state regulations, and community policies, including required documentation, reporting, and follow-up.
- Perform resident assessments as required by state regulations and community policies under the direction of the Director of Assisted Living.
- Observe residents for changes in condition and immediately report concerns to the supervisor and/or Director of Assisted Living.
- Document all pertinent observations, communications with residents' responsible parties, clinical procedures, and daily care activities by the end of each shift.
- Complete and maintain current individualized care plans for residents.
- Assist with other duties as assigned by leadership.
This position works under the supervision of the Director of Assisted Living and/or Director of Memory Care while exercising independent judgment and decision-making. The LPN may assist with the supervision, direction, and evaluation of other associates and may participate in performance reviews and disciplinary actions as needed.
Qualifications- High School Diploma or GED.
- Current Licensed Practical Nurse (LPN) license in the State of Wisconsin.
- Current CPR certification.
- Minimum of one (1) year of related nursing experience.
- Strong communication, organizational, and resident care skills.
- Ability to sit, stand, walk, climb stairs, reach, bend, stoop, crouch, kneel, and perform repetitive movements throughout the workday.
- Ability to lift up to 25 pounds unassisted.
- Ability to assist with carrying, transferring, repositioning, pushing, and pulling residents using proper equipment and assistance as needed.
