Nurse Navigator - Heart Failure Clinic

Role Overview

Provides continuity of care by ensuring smooth transitions across care settings, developing relationships with patients and multidisciplinary teams to facilitate treatment and follow-up.

• Conducts nursing assessments to identify patient needs, collaborates on interdisciplinary care plans, and supports patients and families through education, advocacy, and psychosocial support.
• Coordinates seamless, evidence-based care to achieve optimal patient outcomes, designs and delivers patient education materials and outreach programs, and assists in developing program goals and quality initiatives.

• Bachelor's Degree in Nursing, 5 years of cardiology nursing experience, and current RN licensure.
• Certification in Heart Failure (CHFN) within 2 years and BLS within 6 months required.
• Requires knowledge of heart failure care, strong analytical and communication skills, and ability to collaborate effectively with healthcare professionals and patients.

• Ability to communicate clearly, operate necessary equipment, and adapt to various work environments.
• Flexibility to perform related duties as needed.
