Risk Youth Treatment Liaison

Overview

Professional Services Group is now hiring for the position of Residential Treatment / Foster Care Liaison. We are a community-based social services organization dedicated to supporting families through dynamic and innovative programming responsive to the needs of diverse communities. Our Intensive Aftercare Program (IAP) provides services to families where children are at risk of or have been placed into out-of-home care.

As the Residential Treatment / Foster Care Liaison, you will work directly with youth who have been placed in residential treatment centers, foster care placements, and/or treatment foster care, ensuring they receive the care and support needed to successfully return home. You will collaborate with the youth, their family, and their placement to develop treatment plans that address unique needs and help them achieve their goals.

• Interview and evaluate clients to complete intake process.
• Analyze client needs and strengths to determine services required to meet treatment needs.
• Assist Residential Treatment Center (RTC) and Treatment Foster Care (TXFC) in formulating and evaluating treatment plans.
• Develop aftercare plans, including scheduling reintegration meetings and completing Hillcrest screening.
• Monitor client progress by conducting visits with clients at various institutions at a minimum of once per month.
• Maintain monthly contact with clients  parents/guardians to provide progress updates.
• Coordinate and facilitate a parent trip to each placement at a minimum of once per month.
• Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
• Safely transport clients as needed.

Kenosha, WI. Local travel is required to meet clients and families at home, residential treatment centers, or other placement settings and community spaces (with mileage reimbursement provided).

Monday-Friday 8:00am-5:00pm with occasional evenings to accommodate families  schedules. Rotating Saturdays are required about once or twice per month.

$22.00 per hour

• Must have reliable transportation on a daily basis, a valid driver s license, and automobile insurance.
• Bachelor s Degree in Social Work, Psychology, Criminal Justice, or related field.
