Children’s Therapy Center’s is looking for a Community Resources Navigator to join our team! Children’s Therapy Center’s Community Resources Navigator will work to serve families of children 0-18 years of age across King and Pierce Counties (Washington). The Community Resources Navigator works collaboratively with families to develop a culturally responsive and appropriate individualized plan to access supportive services such as therapy programs, educational programs (Play and Learn Groups, Special Education, Head Start, Community Preschool, ability related services, transitional services), and/or other community programs and resources.
RequirementsPrimary Responsibilities
- Culturally Responsive and Linguistically Appropriate: provide services that are respectful of diverse cultural and linguistic backgrounds and sensitive to the unique needs of each family.
- Resource Navigation: maintain a current directory of community resources and assist families in identifying and accessing resources such as healthcare, disability services, educational support, and more.
- Referral Services: refer families to appropriate community agencies, service providers, and support systems based on their needs.
- Case Management Support: follow-up to ensure families are successfully connected with resources and help them navigate any challenges that arise.
- Family and Community Outreach and Engagement: participate in family and community outreach opportunities, build relationships with community organizations, agencies, and service providers to stay informed about available resources and ensure families are aware of new and updated services.
- Play and Learn Co-Facilitation: regularly co-facilitate Play and Learn sessions, supporting families through structured play-based learning opportunities that promote child development and strengthen caregiver engagement.
- Advocacy and Support: advocate for and facilitate families in self-advocacy to receive the services and support they need, working and address any barriers or obstacles they encounter, and respond to inquiries and requests for information/services in a timely manner.
- Workshops and Educational Support: assist in planning, organizing, and leading play and learn groups, informational sessions or workshops that help families understand how to access and utilize available resources effectively.
- Documentation and Reporting: document and maintain accurate records of services provided, referrals made, and interactions through individualized family plans, ensuring confidentiality, and compliance with Children’s Therapy Center guidelines, policies, procedures, and HIPPA.
- Feedback & Evaluation: collect feedback from participants, track progress, and suggest improvements to enhance the program. Participate in program analysis to review trends, insights, barriers and/or challenges based on caseload reviews and other family data collected through Children’s Therapy Center programs.
- Administrative tasks: participate in monthly caseload and quarterly individualized family plans progress notes reviews to identify program needs, maintain updated outreach materials, conduct research and resource mapping.
- Participate as team member of Children’s Therapy Center; attend relevant meetings and in-services.
- Perform other duties as assigned.
- AA degree in Early Education, Social Work, a related field, or equivalent work experience.
- At least 2 years working within the education, health, and/or human services field at a community based organization.
