Absolute Care is seeking an experienced Advanced Practice Provider to deliver comprehensive outpatient HIV and chronic infectious disease care within a large integrated team. You will develop comprehensive care plans, educate patients, and coordinate services across the continuum of care to improve outcomes.
You will collaborate with social workers, case management, behavioral health, pharmacy, nutrition, and other providers in a PCMH-aligned model, ensuring high-quality, patient-centered care.
