A community-focused home infusion provider delivering safe, compassionate infusion therapy to patients with acute and chronic conditions, including autoimmune, neurological, and inflammatory disorders such as Crohn's disease, ulcerative colitis, and rheumatoid arthritis. The organization is seeking an Infusion Pharmacist to oversee the safe preparation, dispensing, and monitoring of infusion therapies for a diverse patient population, ensuring high-quality, patient-centered care across home and infusion-suite settings.
Whatyou'll do
- Verify medication orders and perform clinical assessments for appropriate dosing and compatibility
- Prepare sterile admixtures in accordance with USP and state board of pharmacy standards
- Collaborate with prescribers, nurses, and other care team members to develop individualized therapy plans
- Provide medication counseling and support transitions of care between home and alternate infusion sites
- Maintain accurate documentation and manage inventory and cold-chain storage
- Ensure regulatory and accreditation compliance
- Participate in quality improvement initiatives to enhance patient safety and outcomes
- Communicate professionally with patients and caregivers to address questions, monitor therapy response, and coordinate follow-up
- Strong clinical pharmacy skills, including infusion therapy management, sterile compounding, dose calculation, and medication monitoring
- Patient care and communication skills, including counseling, education, empathy, and collaboration with multidisciplinary teams
- Organizational and technical skills, including accurate documentation, time management, inventory control, and comfort using electronic health records and pharmacy systems
- Quality and safety-focused skills, including adherence to regulatory standards, infection control practices, and participation in continuous improvement activities
- Doctor of Pharmacy (PharmD) or equivalent degree
- Active and unrestricted pharmacist license in Washington State
- Knowledge of autoimmune, immunology, and chronic disease management, with the ability to interpret clinical lab results and adjust therapy as appropriate
- Ability to work on-site near Kent, WA, maintain professional standards, and provide reliable, patient-centered care in a fast-paced clinical environment
- Prior experience in infusion pharmacy, hospital pharmacy, or home infusion services strongly preferred
