Job Summary:

We are seeking a compassionate and skilled Licensed Social Worker to join our dedicated team of healthcare professionals. As a Licensed Social Worker at Kenton Nursing & Rehab Center, you will play a vital role in providing holistic care to our residents, addressing their emotional, social, and psychological needs. Your expertise will enable you to develop and implement individualized care plans, facilitate communication with residents, families, and interdisciplinary teams, and ensure that each resident receives the highest level of care and support.

Responsibilities:

  • Assess the emotional, social, and psychological needs of residents and develop individualized care plans to address these needs

  • Provide counseling, therapy, and support to residents and their families as needed

  • Facilitate communication and collaboration with interdisciplinary teams, including physicians, nurses, and other healthcare professionals

  • Conduct social worker assessments and make recommendations for care plan changes as needed

  • Document resident information accurately and thoroughly, maintaining confidentiality and adherence to HIPAA guidelines

  • Participate in staff meetings, training, and education programs to stay current with industry best practices and regulations

  • Collaborate with other healthcare professionals to ensure that residents receive comprehensive care and support

Requirements:

  • Current licensure as a Licensed Social Worker (LSW) in the state of Ohio

  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations

  • Ability to work independently and as part of a team to achieve goals and objectives

  • Strong assessment and problem-solving skills, with the ability to think critically and make sound judgments

  • Ability to maintain confidentiality and adhere to HIPAA guidelines

  • Strong organizational and time management skills, with the ability to prioritize tasks and meet deadlines
