We're looking for compassionate, customer-focused professionals to join our team as a Tailored Care Manager Extender! In this role, you'll be the friendly voice on the other end of the phone—connecting members to the care, services, and support they need to live healthier lives.

Make outbound and receive inbound calls with members and healthcare partners.

Build positive relationships through compassionate, professional communication.

Assist Tailored Care Managers by completing outreach, scheduling appointments, coordinating services, and documenting member interactions.

Help connect members with healthcare providers, community resources, and essential support services.

Maintain accurate documentation while meeting quality and productivity goals.

• Excellent communication and active listening skills

• Strong computer and multitasking abilities
• Experience in healthcare, customer service, call centers, or care coordination is a plus
