Responsibilities

Requirements

DDS or DMD degree from an accredited dental school.
Active Arizona Dental License or ability to obtain.
DEA registration or eligibility.
Board certified or board eligible.
Prefer working at an FQHC, CHC, or patient-centered care facility.
