The provider will be covering 4 locations:

• Anderson Hospital
• The Jewish Hospital
• West Hospital

• Practice Setting:
  Hospital, Will not require clinic hours
• Schedule (Shifts and Hours):
  Weekend rounding; 7a-7a
• Average Number of Cases Per Day:
  Rounding at 4 hospitals with an average of 6 patients at each hospital
• Documentation System/EMR: EPIC
• Trauma Level/Designation: 1
• Number of Operating Rooms:
  Each hospital has multiple
• Patient Age Demographic:
  Adult

• Aneurysm repairs
• Abdominal
• Arterial bypass
• Peripheral
• Vascular Access Devices
• AV shunt

• Bypass Surgery
• Femoral Popliteal Bypass
• Aortofemoral Bypass
• Stenting
• Endovascular and Open Surgical Reconstruction for Deep Vein Occlusions

• Vascular Malformations
• Nutcracker Syndromes
• Lymphedema
• Gangrene of the limbs
• Peripheral Artery Disease

• ~$150/night lodging cap
• All other bookings must be standard/economy

• Board Certified Vascular Surgeon

• Fellowship Training

• OH License
