Per Diem Geriatric Physician

St. Joseph Hospital – Nashua, New Hampshire

St. Joseph Hospital, a member of Covenant Health, is seeking an experienced Board-Certified/Board-Eligible Geriatric Physician to join our growing Senior Care team in Nashua, New Hampshire on a Per Diem basis.

This is an outstanding opportunity to provide comprehensive care to older adults across multiple care settings while enjoying a flexible schedule and a collaborative, mission-driven environment.

Join an established geriatrics team consisting of a physician leader and experienced advanced practice providers dedicated to improving the health and quality of life of older adults throughout Southern New Hampshire.

• Geriatric outpatient services

• Skilled nursing and rehabilitation facilities

• Long-term care facilities

• Post-acute care settings

The team is experiencing significant growth, with patient visits and productivity exceeding budget expectations. Additional expansion opportunities include growth of post-acute services and potential Medical Directorship opportunities.

• Per Diem opportunity

• Day shift schedule

• Outpatient-focused practice with long-term care and rehabilitation responsibilities

• Participate in Internal Medicine call rotation

• First-call coverage managed through RN triage

• Established team of physicians and advanced practice providers

• Opportunity to care for a diverse geriatric population across multiple care settings

• Potential leadership and Medical Directorship opportunities

• Board Certified or Board Eligible in Geriatric Medicine, Internal Medicine, or Family Medicine

• Minimum of five years of clinical experience preferred

• Clinical interest, training, or certification in Gerontology strongly preferred

• Strong commitment to quality, patient-centered care

• Ability to work collaboratively within an interdisciplinary team

The Senior Care team currently includes:

• Physician Medical Director

• Two experienced Advanced Practice Providers

The department cares for an average of more than 450 patients per month and continues to expand services throughout the region.
