Your Impact

As an Advanced Practice Provider, you’ll provide comprehensive, interdisciplinary primary care to a dedicated panel of patients. You’ll have the opportunity to care for patients across both routine and acute needs while partnering with the broader care team to improve outcomes.

Your responsibilities will include:

  • Providing comprehensive primary care through in-person and telehealth visits.
  • Managing routine, acute, and chronic medical conditions.
  • Completing comprehensive Annual Wellness Visits and Health Risk Assessments.
  • Working collaboratively with physicians and interdisciplinary team members to coordinate care and address the whole person’s health needs.
