Responsibilities And Duties Include But Are Not Limited To- Provides intensive social work services for individuals under state guardianship to include making medical, personal, and or financial decisions.
- Provides support and services to complex cases, including placements within appropriate residential or community-based settings.
- Makes face-to-face visits with clients to assess needs and living conditions and assist with access to community resources. This includes residence, respite, or hospitalization, which may lead to exposure to contagious diseases. This includes individuals experiencing severe problems, which jeopardize placement or may lead to hospitalization. Help move belongings, including heavy objects.
- Prepares court reports and monitors conditions as set by court appointments. Works to provide intensive social work services to those complex cases appointed by the court, including those with severe mental illness and those with no resources for housing or personal needs.
- Works with residential providers, fiduciary staff to ensure payment of bills and provision of personal needs, and benefits staff to establish and maintain benefits, including Medicaid, food stamps, pensions, and federal and other eligible benefits. Responsible for entering data into the Kentucky Guardianship Fiduciary Information System (KYGFIS) and maintaining records.
- Works as a member of the service plan review team to establish a plan of care. Ensures unified service provisions as developed on the plan of care, including monitoring of clients' activities of daily living and community involvement. Provides care coordination and advocacy for clients under Guardianship. Provides expert court testimony and works with referral sources to provide education regarding Guardianship to ensure the referrals are appropriate prior to petitioning the court for an appointment.

EDUCATION: A master's degree in social work, sociology, psychology, marriage and family therapy or a related field.
EXPERIENCE, TRAINING, OR SKILLS: Must have two years of professional social work experience.
Substitute EDUCATION for EXPERIENCE: NONE
Substitute EXPERIENCE for EDUCATION: A bachelor's degree supplemented by four years of professional social work experience will substitute for the primary education and experience requirements.
