Job Summary/Purpose

The Clinical Dietitian is responsible for providing evidence‑based medical nutrition therapy (MNT) to patients in the hospital setting, supporting optimal health outcomes through individualized nutrition assessments, interventions, and education. This role is integral to ensuring compliance with TJC, CMS, state health department, and Academy of Nutrition and Dietetics standards. The Dietitian collaborates with the interdisciplinary care team—including physicians, nurses, case managers, therapists, and food service personnel—to support quality patient care, regulatory compliance, and hospital accreditation readiness.

  • Conduct comprehensive nutrition assessments for patients identified through screening protocols, physician referrals, or regulatory requirements.
  • Develop and implement individualized nutrition care plans based on clinical condition, lab values, treatment goals, cultural considerations, and regulatory standards.
  • Monitor patient progress and document assessments, interventions, and outcomes in the electronic health record (EHR) per hospital policy and regulatory guidelines.
  • Provide evidence‑based nutrition recommendations to physicians and the care team.
  • Adjust diet orders in accordance with hospital policy, medical staff‑approved protocols, and CMS/TJC requirements.
  • Ensure nutrition care practices comply with:
  • TJC standards (e.g., PC – Provision of Care, LD – Leadership, IC – Infection Control).
  • CMS Conditions of Participation, particularly §482.28 (Food and Dietetic Services) and §482.23 (Nursing Services related to nutritional needs).
  • State licensing and local public health regulations.
  • Participate in survey readiness activities, tracer rounds, and performance improvement initiatives related to nutrition services.
  • Ensure compliance with standards for safe food handling, dietary management, and infection control practices in patient care.
  • Provide nutrition education and counseling to patients and families on therapeutic diets, disease management (e.g., diabetes, heart failure, renal disease), and discharge planning.
  • Document education provided and assess comprehension to support continuity of care and regulatory requirements.
  • Develop and update patient education materials consistent with hospital‑approved guidelines.
  • Actively participate in daily interdisciplinary rounds, care conferences, and discharge planning meetings.
