The AOT Coordinator provides case management services to assigned consumers and ensures adherence to all Pathways policies and procedures. This role focuses on in-reach, offering engagement, education, and support to adults with Serious Mental Illness (SMI) who reside in personal care homes, facilities, institutions, or who are experiencing homelessness. The AOT Coordinator plays a critical role in connecting individuals to community-based mental health services, housing options, and court-ordered treatment supports.

Certification / License RequirementsPosition may be filled by a Peer Support Specialist (PSS) or Targeted Case Manager (TCM)
Duties & Responsibilities- Provide case management services to assigned consumers
- Conduct in-reach to engage adults with SMI in facilities, institutions, and community settings
- Educate consumers on available mental health services and housing options
- Support compliance with Assisted Outpatient Treatment (AOT) requirements
- Collaborate with courts, providers, and community partners as appropriate
- Maintain accurate, timely documentation in AVATAR and other required systems
- Meet quality standards, productivity expectations, and reporting deadlines
- Adhere to all Pathways policies, procedures, and regulatory requirements
- Experience working with individuals diagnosed with Serious Mental Illness (SMI)
- Knowledge of, or willingness to learn, the judicial system as it relates to AOT
- Strong communication and interpersonal skills
- Ability to meet deadlines and quality assurance standards
- Proficiency with computers, electronic documentation, and electronic devices
- Experience in a Behavioral Health setting preferred
