Job Summary

We are seeking a full‑time Home Health Physical Therapist to support patients in Middleburg, PA and surrounding areas. The role focuses on collaborating with a team of professionals to manage and deliver patient care plans, ensuring high‑quality service and efficient clinical outcomes.

• Provide assessment, treatment, and ongoing evaluation of patients in their homes.

• Develop and execute individualized care plans in partnership with a multidisciplinary team.

• Maintain accurate and timely clinical documentation in EMR systems.

• Coordinate care and communicate progress with patients, families, and other health‑care professionals.

• Travel to patient homes as needed (≈20% of the workday).

• Meet critical charting and reporting deadlines.

• Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.

• Licensed Physical Therapist licensed in the state of practice.

• Minimum of one year of health‑care experience.

• Valid driver’s license with ability to drive to patient sites.

• Strong time‑management and proficiency with tablets, mobile phones, and EMR software.
