We are hiring Direct Support Professionals for our Day Habilitation Services in Queens.

YAI’s day programs are designed to assist people to acquire, retain or improve their self-help, socialization and adaptive skills, including communication, travel and other areas in adult education. Activities and environments are designed to foster the development of life skills and adaptation to appropriate social behavior; all to achieve greater independence.

The day program focuses on community inclusion through volunteer work, healthy relationship building, promoting self-advocacy and making informed decision.

Key Responsibilities
  • To provide direct care services as needed. This includes, but is not limited to, the provision of counseling, training in ADLs (Assisted Daily Living skills), assistance with personal care, medical, dietary, social, sexual, recreational, financial, habilitative and other needs within the program.
  • Ensure the health, safety, and wellness of the people we support.
  • Actively carry out progressive teaching style, which is designed to measure accuracy, fluency, and competency on established individualized goals using foundational Teaching skills (EMOE, Task analysis, essential skills, supports and modifications, etc.).
  • Maintain a conducive learning environment through a variety of methods:
    Personal Outcome Measures, Person Centered Thinking, Informed Decision Making.
  • Complete thorough and timely monthly documentation to ensure regulatory compliance and meet billing deadlines.
  • Provide timely services that are in accordance with everyone’s set schedule so as to ensure consistency and the respect of each person receiving services.
  • Be responsible for the implementation of the day program plans, opportunities for self-determination, independent activity, increased productivity and greater community inclusion.
  • Collect, record, and report data for all written program plans as assigned (i.e. behavior plans, treatment plans, staff action plans, etc.). Write case notes and provide information on incidents as required.
  • Demonstrate a high level of communication skill through articulate written and verbal modes, being able to effectively relay detailed, important information to all stakeholders.
  • Complete mandatory trainings based on the program and position (CPR/First Aid, SCIP, etc.), and apply the learned skills.
  • Ensure appropriate supervision of the people we support, the program – and understand when to contact an administrator for support.
  • Escort people on recreational activities, volunteer sites, and various other community events and/or functions.
  • Assist people with making purchases, as well as safeguard personal allowance funds and any additional funds.
  • Take on additional tasks outside of basic DSP work and complete job related tasks as needed.
  • Strong writing skills required; an ability to analyze, interpret, and summarize data through detailed progress notes.
Education and Experience
  • High School Diploma or GED preferred.
  • Work or life experience in the I/DD field is a plus, but not mandatory (for DSP 1 positions). For DSP 2 positions, at least three years of Direct Support Professional experience in the I/DD field.
  • Valid NY State Driver’s License is a plus.
  • Tuberculosis test is mandatory for most positions.
Schedule
  • Full time;
    Monday through Friday
  • 8:00 AM to 4:00 PM (Bayside location)
  • 7:30 AM to 3:30 PM (Kew Gardens location)

Compensation
  • $16.78/hour to $16.78/hour
