Employment Type: W-2

Schedule & Coverage

Flexible scheduling available:

  • Part-Time: 2 days per week (you choose the days)
  • Full-Time: Monday–Friday daytime hours (typically one facility per day)
    Full-time employees are eligible for the full benefits package.

Service Areas May Include: Paducah, Bardwell, Mayfield, Clinton, Fulton, Hopkinsville, Greenville, and Monticello, KY

Job Summary

Telos Health Systems is seeking a Psychotherapist (LCSW, LPCC, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services to patients in Skilled Nursing Facilities and Assisted Living Facilities throughout the Clinton, KY area. This role offers flexibility, clinical autonomy, and strong administrative and leadership support.

Key Responsibilities
  • Conduct comprehensive mental health assessments and diagnostic evaluations
  • Provide individual psychotherapy and ongoing treatment services
  • Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team
  • Maintain accurate, timely, and compliant clinical documentation
  • Coordinate care with facility staff, medical providers, and other healthcare professionals
  • Adhere to all federal, state, and organizational standards, policies, and regulations
Qualifications Required
  • Active Kentucky license as one of the following:
    • Licensed Clinical Social Worker (LCSW)
    • Licensed Professional Clinical Counselor (LPCC)
    • Licensed Marriage and Family Therapist (LMFT)
    • Licensed Psychologist (PsyD or Ph.D.)
  • Master’s degree (LCSW, LPCC, LMFT) or Doctoral degree (Psychologist) in a relevant mental health field
  • Ability to work independently while collaborating effectively within a clinical team
  • Strong communication and clinical documentation skills
Preferred
  • Experience working with geriatric populations
  • Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings
