Location: Lewisburg
Residential Home Health and Hospice is seeking a dedicated Physical Therapist to join our Home Health Admissions team in Williamsport, PA. You will manage patient care plans, collaborate with the home health team, and ensure timely charting and documentation for residents in Williamsport and surrounding areas.
The role requires a licensed PT in the state, a degree in physical therapy, and at least one year of healthcare experience. Travel up to 20% and proficiency with tablets/EMR are preferred.
