Targeted Case Manager

Russell Springs Elementary Healthy Kids Clinic

Targeted case management (TCM) services is a nonclinical service that assists the recipient in gaining access to needed resources. The Targeted Case Manager works to create and coordinate partnerships with formal and informal resources. The overall goal is to assist individuals with disabilities to live successfully in their home and community. The Targeted Case Manager works to confront barriers and coordinate services.

They ensure independence and patient choice while employing a strength-based approach toward understanding the recipient’s beliefs and behaviors as they work toward achieving projected goals.

Job Functions/Responsibilities
  • Coordinating the development of effective service plans through optimizing the recipient’s personal and community resources.
  • Performing a comprehensive assessment and periodic reassessments of the recipient's needs for medical, educational, social, or other services.
  • Promote multi-disciplinary care that is patient-centered and considers all aspects of recipient’s personal, psychological, economic, and cultural needs. Monitor and track service delivery and provide advocacy consistent with the recipients need and or barriers to service.
  • Maintains thorough knowledge of CFMC services/benefits, public/private community resources, as well as federal/state laws which affect service planning.
  • Completes required documentation of services and plan of care per policy.
  • Other duties as assigned.
Minimum Qualifications
  • One (1) year of TCM experience providing services to the identified population.
  • Have a Bachelor of Arts or science degree in behavioral health science such as psychology, sociology, social work, family studies, human studies, counseling, nursing, behavioral analysis, public health, special education, gerontology, recreational therapy, education, occupational therapy, physical therapy, speech language pathology, rehabilitation counseling, or faith-based education.
  • OR be a certified alcohol and drug counselor with any Bachelor of Arts or science degree.
  • AND Complete required case management training i.e. 12 hours of core training with an additional 6 hours of training consistent with the population group served, i.e. children with a severe emotional disability, individuals with a severe mental illness, individuals with a substance use disorder and/or individual with complex physical health issues paired with a substance use or mental health disorder.
Additional Requirements
  • Ability to work effectively in a team environment
  • Work hours are congruent with the needs/requirements of the position
  • Provide transportation to individuals/family members(s) served through TCM services.
Preferred Qualifications
  • Five (5) years’ experience providing targeted case management services to the identified population.
  • Demonstrated ability to provide leadership.
  • Advanced knowledge of local, state and national resources related to service planning.
Schedule Requirements

Full-time position, 5 days per week, 100% clinic-based, in person work required. Work schedules are set at the discretion of CFMC and may be adjusted at any time based on the operational needs of the organization.
