Signature HealthCARE is seeking a Social Services Director for Rockcastle Health & Rehabilitation Center in Kentucky to identify and address residents' social, emotional, and psychological needs while coordinating discharge planning.
You will lead care planning, support residents and families, and ensure timely documentation to comply with Facility, State, and Federal policies. The role requires a Bachelor's or Master’s in Social Work or Psychology and 3+ years in Social Services.
