Full Time – LCSW / LPCC / LMFT

• Provides comprehensive psychosocial assessments and behavioral health care.
• Uses age‑appropriate assessment and intervention skills and a range of interventions based on patient needs.
• Assists members in developing cost‑effective, appropriate health care plans by optimizing personal and community resources.
• Promotes multi‑disciplinary, patient‑centered care that considers personal, psychological, economic, and cultural needs.
• Maintains thorough knowledge of CFMC services, community resources, and applicable federal/state laws affecting health care planning.
• Completes required documentation of services and plans of care per policy.
• Participates in program development, orientation, and educational activities to build members' capacity for self‑directed health care.
• Other duties as assigned.

• One (1) year of experience in mental health, hospital, medical outpatient clinic, FQHC, home health, or long‑term care setting.
• Three to five (3‑5) years of experience in a related environment.
• Master’s degree or equivalent with appropriate licenses, certifications, and registrations.
• Current unrestricted license in the state of practice, required upon hire.
• Basic Life Support (BLS) for Health Care Providers.
• National Provider Identifier (NPI) required prior to employment start date.

• Expertise with brief counseling interventions.
• Demonstrated ability to provide leadership.
• Advanced knowledge of local and national resources related to behavioral health care.

Full‑time position, five days per week, 100% clinic‑based, in‑person work required. Work schedules are set at the discretion of CFMC and may be adjusted at any time based on operational needs.
