Job Overview
Cumberland Family Medical Center (CFMC) operates a Federally Qualified Health Center (FQHC) that offers a wide range of health and mental health services at multiple sites throughout Kentucky. LocationFirst Choice Immediate Care – McCreary, Whitley City, KY 42653
PositionFull Time – LCSW / LPCC / LMFT
Responsibilities- Provides comprehensive psychosocial assessments and behavioral health care.
- Uses age‑appropriate assessment and intervention skills and a range of interventions based on patient needs.
- Assists members in developing cost‑effective, appropriate health care plans by optimizing personal and community resources.
- Promotes multi‑disciplinary, patient‑centered care that considers personal, psychological, economic, and cultural needs.
- Maintains thorough knowledge of CFMC services, community resources, and applicable federal/state laws affecting health care planning.
- Completes required documentation of services and plans of care per policy.
- Participates in program development, orientation, and educational activities to build members' capacity for self‑directed health care.
- Other duties as assigned.
- One (1) year of experience in mental health, hospital, medical outpatient clinic, FQHC, home health, or long‑term care setting.
- Three to five (3‑5) years of experience in a related environment.
- Master’s degree or equivalent with appropriate licenses, certifications, and registrations.
- Current unrestricted license in the state of practice, required upon hire.
- Basic Life Support (BLS) for Health Care Providers.
- National Provider Identifier (NPI) required prior to employment start date.
- Expertise with brief counseling interventions.
- Demonstrated ability to provide leadership.
- Advanced knowledge of local and national resources related to behavioral health care.
Full‑time position, five days per week, 100% clinic‑based, in‑person work required. Work schedules are set at the discretion of CFMC and may be adjusted at any time based on operational needs.
