LIFE COORDINATED is seeking a Licensed Clinical Social Worker to join the interdisciplinary care team serving elders in Kentucky. The role offers part-time or full-time options, with a salary range of $52,000–$72,800.
The LCSW will advocate for participants, coordinate with families and IDT, and ensure comprehensive social work care planning, discharge planning, and access to Medicaid/Medicare resources.
