Physical Therapist (PT) – Part-Time or PRN

Position Responsibilities
  • Evaluate residents and develop individualized treatment plans

  • Provide skilled physical therapy services in a SNF/LTC environment

  • Collaborate with nursing, therapy, physicians, and interdisciplinary team members

  • Monitor resident progress and adjust treatment plans as needed

  • Complete timely and accurate documentation using Net Health

  • Promote resident mobility, safety, independence, and quality of life

Qualifications
  • Graduate of an accredited Physical Therapy program

  • Current Pennsylvania PT License or license eligibility

  • Strong communication and interpersonal skills

  • Ability to work effectively as part of a multidisciplinary team

  • Passion for delivering exceptional resident-centered care

  • SNF/LTC experience preferred but not required

  • New Graduates Welcome to Apply
