Responsibilities
The Social Worker is a member of the multidisciplinary care team to meet the following patient/family significant other needs: response to psychosocial and psychodynamic needs; coordination of community resources to ensure the transition to the least restrictive care environment, coordination/facilitation of discharge planning, seamless continuity of care process, therapeutic intervention and financial counseling. The Social Worker interfaces directly with patient/family/significant other and all other interdisciplinary members of the care team.
The Social Worker provides short‑term counseling and crisis intervention to patients and families identified as having complex social services needs, including but not limited to; emotional/psychiatric issues, family violence or abuse, legal issues or financial problems.
Required:
- One year social work or One year social work internship.
- Master Degree
- Licensed Clinical Social Worker
Desired:
- One year in a healthcare setting;
Two years in Psych setting
