Breakthrough Healthcare is seeking responsible, dependable Home Health Aides (HHAs) who are confident working independently and committed to delivering high‑quality, compassionate care in the home setting.

We are seeking HHAs who are:

• Responsible and able to work without direct supervision
• Punctual, reliable, and professional
• Compassionate and respectful in all interactions
• Committed to providing care with integrity and dignity
• Interested in learning, growing, and developing professionally
• Comfortable working independently in clients’ homes

• Assist clients with activities of daily living (bathing, dressing, grooming, toileting)
• Provide mobility assistance and support with transfers
• Observe and report changes in client condition to the care team
• Maintain a clean, safe, and supportive home environment
• Offer companionship and emotional support
• Follow individualized care plans and agency policies
• Document care accurately and in a timely manner
• Uphold client privacy, safety, and dignity at all times

• Active HHA certification (or equivalent experience, where applicable)
• Prior experience in home care or facility‑based care preferred
• Ability to work independently and manage time effectively
• Strong communication and interpersonal skills
• Reliable transportation
