* Qualifications:

*** PharmD from a College of Pharmacy accredited by American Association of Colleges of Pharmacy, or equivalent clinical experience required.
* Current Montana State Board of Pharmacy, Registered Pharmacist License required.
* Accurate data entry skills, analytical and advanced math skills required.
* RQI Provider (BLS) within 7 days of hire/transfer or before independent patient care, whichever comes first, required.
* First Responder required for assignment:
Clinic, Retail, Behavioral Health, IS and Administration Pharmacists. ACLS required for all other assigned area(s).
* Demonstrated ability to make independent judgments under stressful situations at times.
* Must work effectively and efficiently without direct supervision.

* Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.

Specific Duties :
*** Evaluates and verifies orders for therapeutic appropriateness, proper dosing, optimal dosage form, drug interactions, allergic reactions, route and rate of administration, and therapeutic redundancy.
* Provides patient-specific pharmacotherapy recommendations and drug information to the appropriate stakeholders as applicable to assigned area(s).
* Performs daily review of pharmacy reports including pharmacokinetic monitoring as applicable to assigned area(s).
* Documents clinical interventions appropriately in electronic medical record.
* Implements all drug use policy and formulary decisions of the Pharmacy and Therapeutics committee.
* Reviews and updates Collaborative Practice Agreements per department protocol for assigned area(s).
* Supports and precepts pharmacy students on practice rotations as applicable to assigned area(s).
