Signify Health, part of CVS Health, is seeking a Full Time Clinician (NP or PA) to perform in-home health evaluations. Visit members at home, review meds and history, and perform clinical assessments using an iPad workflow. Travel locally within licensed states and also conduct virtual visits as needed.
Required:
active, unrestricted NP/PA license(s) and board certification; multi-state licenses preferred. Anticipated weekly hours are 40, with a comprehensive benefits package for full-time staff.
