Cordova Wellness & Rehab is seeking a compassionate, reliable Social Services Director to join our care team. If you're looking for a rewarding role in a team-driven environment, we want to meet you!

As the Social Services Director at Cordova Wellness & Rehab, you'll be an essential part of our residents  care.

Responsibilities include:

• Oversee the development, implementation, supervision, and ongoing evaluation of the Social Services Department.
• Meet with and assist residents and their families with medically related services, grievances, or other placement needs to help maintain the highest quality of care.
• Complete assessments and participate in comprehensive care plans.
• Facilitate residents’ safe transition upon discharge as well as facilitate transportation for appointments or other scheduled events.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.
• Perform all other assigned tasks as directed by the Administrator, Director of Nursing, or corporate team.

• A minimum of a bachelor’s degree in social work or another human services field such as sociology, gerontology, special education, rehabilitation counseling, or rehabilitation or be a licensed Social Worker
• 1 year of supervised social work experience in healthcare
