Location: Northern

We are seeking a licensed Physical Therapist (PT) to join our skilled nursing team in Marion, SC. This is an excellent opportunity for a clinician who values patient-centered care, clinical autonomy, and collaboration within a supportive interdisciplinary environment.

Position Summary

The Physical Therapist evaluates, plans, and implements individualized treatment programs for residents in a skilled nursing setting, helping patients regain strength, mobility, and functional independence following illness, injury, or surgery.

Key Responsibilities

  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
  • Provide skilled PT services to short-term rehab and long-term care residents.
  • Establish measurable goals and monitor patient progress.
  • Document evaluations, treatment notes, and progress reports in accordance with regulatory standards.
  • Collaborate with physicians, nursing staff, therapy team, and families to optimize patient outcomes.
  • Participate in care plan meetings and discharge planning.

    Ensure compliance with Medicare, Medicaid, and facility policies.

Qualifications

  • Current Physical Therapist license in the State of South Carolina.
  • Graduate of an accredited Physical Therapy program.
  • Experience in a skilled nursing or rehab setting preferred (new grads encouraged to apply).
  • Strong clinical, documentation, and communication skills.
  • Ability to work independently while contributing to a team-based environment.
