Prestige Care of Fairfield is a leading provider of subacute rehabilitation and skilled nursing care in the community. We are dedicated to improving the quality of life for our residents by delivering compassionate, evidence-based therapy services in a supportive and collaborative environment.
Position SummaryWe are seeking a licensed Physical Therapist (PT) to join our in-house rehabilitation team. In this role, you will provide high-quality, individualized therapy services to residents in our subacute rehabilitation unit, helping them achieve their highest level of function and independence. Flexible scheduling is available, with both full-time and per diem opportunities.
Key ResponsibilitiesEvaluate, plan, and implement individualized physical therapy programs for residents
Provide therapeutic interventions including mobility training, balance exercises, strengthening, and gait training
Monitor and document resident progress, updating care plans as appropriate
Collaborate with physicians, nursing staff, occupational therapy, and speech therapy to ensure coordinated care
Educate residents and families on exercises, adaptive techniques, and safe mobility
Participate in team meetings and quality improvement initiatives
Maintain compliance with federal, state, and facility regulations
Licensed Physical Therapist in the state of Iowa
Minimum of 1 year of experience in a skilled nursing or subacute rehabilitation setting preferred (new graduates welcome)
Strong clinical reasoning, communication, and documentation skills
Ability to work independently and as part of a multidisciplinary team
CPR certification required or willingness to obtain
Per Diem: Flexible scheduling based on facility needs
Premium pay rates up to $75/hour
