PRIMARY FUNCTION The Community Health Worker works as part of the primary care team to support patient understanding of chronic conditions and to encourage a healthy lifestyle. The Community Health Worker will facilitate a shared goal model to achieve care that is patient and family centered. Additionally, the Community Health Worker will assist patients in coordinating care across multiple care delivery settings and in accessing community resources to overcome barriers in improving health.

PRINCIPLE DUTIES General

Participate as a key member of the care team, including participating in the morning huddle and other team based activities.

Maintain communication with the care team by documenting conversations with and/or relating to patient in the electronic health record.

Assist with other duties or in other departments as instructed by supervisor.

Health Education Assess patient’s health literacy to facilitate selection of appropriate education interventions.

Receive health education referrals from providers. Complete approved disease management or healthy behavior curricula with patients.

Assist patients in setting self-management goals utilizing motivational interviewing techniques.

Monitor patient progress on plan of care and facilitate changes as needed.

Remain informed of approved tools and documents that support a guided care process.

Care Coordination Cultivate and support primary care and subspecialty co-management with timely communication, inquiry, follow up, and integration of information into the care plan regarding transitions-in-care and referrals.

Coordinate communication with patient and care team after inpatient discharge and/or emergency department visits.

Reinforce patient understanding of appropriate care setting if needed.

Serve as a key contact and resource for family members and other care providers for high-risk patients.

Facilitate and attend meetings between patient, families, care team, payers and community resources, as needed.

Community Resource Navigation Assess patient and family’s unmet health and social needs.

Provide navigation assistance with community resources such as food/nutrition, housing, advocacy, health care navigation, job searching, transportation assistance and any other non-medical need.

Complete referrals to community resources as required.

Ensure effective tracking of test results, medication management, and adherence to follow-up appointments by working with appropriate health information staff.

Utilize community resources lists, submit updates to the Enabling Services SLM, and serve as a reference to other care team members on community resources.

ACCOUNTABILITY Accountable to the Manager of Patient Support Services.

TYPICAL WORKING CONDITIONS Work is performed in office environment and involves frequent contact with staff and the public. Position may involve dealing with angry or upset people. Evening and/or weekend work is required. Work may be stressful VEL REQUIREMENTS Travel within the community may be required to support patient care coordination and community resources needs.

WORK HOURS
