• Perform comprehensive PT evaluations and develop a plan of care for SNF residents.
• Implement evidence-based interventions: therapeutic exercise, gait training, balance, ROM, and pain management.
• Reassess progress regularly, adjust treatment plans, and document outcomes in resident records.
• Collaborate with nursing and rehab teams; contribute to discharge planning and caregiver education.
• Ensure patient safety, maintain HIPAA-compliant documentation, and comply with facility policies and regulations.

• Active physical therapy license in good standing in the practice state or eligibility for licensure transfer per assignment policies.
• Strong clinical assessment, documentation, and communication skills.
• Ability to work on-site in a skilled nursing facility setting.
• CPR/BLS certification.
• Ability to travel and commit to a 13-week assignment.
• Flexible, team-oriented approach and reliable attendance.
