Center Well Home Health is seeking a caring Home Health Physical Therapist Assistant to deliver PT services in patients' homes, under the supervision of a Physical Therapist. You will implement treatment plans, perform therapeutic exercises, document health records, and participate in team in-services.
Requirements include a current Physical Therapy license, CPR, a valid driver's license with insurance, and English fluency. Prior geriatric experience and teamwork are highly valued.
