Social Worker, Crisis Counselor

Interim Health Care seeks a compassionate Social Worker MSW to support patients receiving home health, hospice, and personal care services. In this field-based role, you will complete psychosocial assessments, develop care plans, and provide counseling, crisis intervention, and resource coordination for patients and families in their homes. Responsibilities

• Provide psychosocial assessments for home health and hospice patients
• Develop and implement individualized care plans and interventions
• Coordinate community resources and support services for patients and families
• Offer counseling and crisis intervention to patients and caregivers
• Collaborate with nurses, therapists, and physicians on care planning
• Advocate for patient rights, safety, and access to services
• Educate families on coping strategies, benefits, and care options
• Maintain accurate, timely clinical documentation and reports
• Participate in interdisciplinary team meetings and case conferences
• Support end-of-life planning and bereavement resources as needed
