Social Service Assistant

The Greens at Cabarrus
Concord, North Carolina

Job Summary:

The Greens at Cabarrus is a long-term care facility dedicated to providing individualized care and support to our residents. We are seeking a caring, organized, and dependable Social Service Assistant to join our team. As a Social Service Assistant, you will play a vital role in supporting our residents and their families by addressing their emotional, social, and practical needs.

Key Responsibilities:
  • Assist with resident assessments, care planning, and discharge planning

  • Help residents and families adjust to admission and long-term care

  • Maintain progress notes, reports, and other required documentation

  • Provide information about resident rights, financial assistance programs, and community resources

  • Connect residents and families with appropriate community agencies and services

  • Assist with scheduling appointments and arranging transportation

  • Maintain regular communication with residents and their families

  • Help address resident concerns and grievances according to facility procedures

  • Collaborate with nursing, therapy, medical, and other facility departments

  • Participate in care-plan meetings, training programs, and required in-services

  • Protect resident privacy and confidentiality in accordance with HIPAA and facility policies

  • Perform other related duties as assigned

Qualifications:
  • High school diploma or equivalent required

  • Previous social services or long-term care experience preferred, but not required

  • Strong communication, organizational, and documentation skills

  • Compassionate and patient when working with residents and families

  • Ability to manage sensitive and emotional situations professionally

  • Ability to work both independently and as part of an interdisciplinary team

  • Basic computer skills

  • Ability to meet the physical requirements of the position, with or without reasonable accommodation
