Care Pavilion Nursing & Rehabilitation Center in Philadelphia, PA is seeking a Full-Time Physical Therapist (PT) to join our in-house rehabilitation team
. We also have PRN opportunities available for therapists looking for additional flexibility.

As part of our in-house rehab department, you'll have the opportunity to work closely with nursing, therapy, and facility leadership while providing individualized care to our skilled nursing and long-term care residents.

As a Physical Therapist, you will:

• Evaluate residents and develop individualized plans of care

• Provide skilled physical therapy services to SNF/LTC residents

• Help residents improve strength, mobility, balance, safety, and functional independence

• Establish measurable functional goals and monitor resident progress

• Complete accurate and timely documentation

• Collaborate with interdisciplinary team members to support positive resident outcomes

• Provide resident, family, and caregiver education as appropriate

• Graduate of an accredited Physical Therapy program

• Current Pennsylvania Physical Therapist license or eligibility for licensure

• Strong communication and clinical skills

• Ability to work effectively as part of an interdisciplinary team

• SNF/LTC experience is a plus

• Experience with
  Net Health and Point Click Care  (PCC) is a plus
