* Job Description:

** The H.O.P.E. (Helping Others Progressively Evolve) Program Case Manager provides intensive, participant-centered case management for adults involved in the criminal justice system who have behavioral health, substance use, and co-occurring needs. The Case Manager serves as the primary point of contact for Dallas County Criminal Justice participants enrolled in the H.O.P.E. Program.

The Case Manager conducts comprehensive assessments, develops individualized service plans, coordinates access to behavioral health and community-based services, and builds collaborative treatment teams that include participants, family and natural supports, behavioral health providers, probation officers, attorneys, peer recovery specialists, and community partners. The Case Manager promotes participant engagement, accountability, recovery, and successful community reintegration through coordinated, evidence-based case management.

This position requires a professional who can build trusting relationships while maintaining clear professional boundaries and supporting the expectations of a specialty program. Using knowledge of Social Drivers of Health, Trauma-Informed Care, Recovery-Oriented Systems of Care, Harm Reduction principles, and Motivational Interviewing, the Case Manager coordinates services that address participants' behavioral health, substance use, housing, employment, education, transportation, healthcare, recovery support, and other social service needs while promoting long-term stability and reducing recidivism.
** RESPONSIBILITIES:
*** Serve as the primary point of contact for H.O.P.E. Program participants, providing ongoing support, advocacy, engagement, and care coordination throughout program participation.
* Conduct comprehensive behavioral health, psychosocial, and resource assessments to identify participant strengths, needs, barriers, and recovery goals.
* Develop, implement, monitor, and update individualized, person-centered service and recovery plans that support treatment participation and successful completion of program requirements.
* Utilize Motivational Interviewing, strengths-based practices, trauma-informed care, and crisis intervention techniques to promote engagement, accountability, behavior change, and recovery.
* Maintain regular contact with participants through office visits, telephone contacts, community visits, and collateral contacts as appropriate.
* Coordinate behavioral health treatment, substance use treatment, psychiatric services, medical care, medication management, peer recovery support, and other community-based services.
* Facilitate referrals, coordinate appointments, monitor attendance, and follow up to ensure successful linkage with recommended services.
* Coordinate continuity of care during transitions between incarceration, residential treatment, outpatient treatment, hospitalization, community supervision, and recovery support services.
