Element Care seeks a Director of Behavioral Health to lead integrated mental health services for frail seniors enrolled in our PACE program. The Director will design and oversee behavioral health strategies, manage clinical staff, and ensure high-quality, patient-centered care across day centers, clinics, and home visits. Responsibilities include program development, supervision, regulatory compliance, crisis management, and collaboration with medical, nursing, and social work teams.
This role champions dignity, respect, and independence for elders while advancing innovative community-based care models and supporting staff development.
- Lead the design and implementation of behavioral health strategies for PACE participants.
- Oversee and mentor behavioral health clinicians, providing clinical supervision and performance management.
- Ensure compliance with state, federal, and PACE regulations and behavioral health best practices.
- Collaborate with medical, nursing, social work, and rehab teams to deliver integrated, patient-centered care plans.
- Develop and monitor quality metrics, outcomes, and continuous improvement initiatives for behavioral health services.
- Manage behavioral health crisis response, risk assessment, and safety planning for frail seniors.
- Participate in case conferences and interdisciplinary team meetings to coordinate care across settings.
- Develop policies, procedures, and workflows that support community-based, culturally competent care.
- Support staff education, training, and professional development in behavioral health topics.
- Contribute to organizational strategy and innovation in geriatric and community-based behavioral healthcare.
