Position Summary

Baptist Health Brookwood Hospital

Ensures that the medically related emotional and social needs of the residents are met to allow them to attain or maintain their highest practicable physical, mental, and psychosocial well-being.

Serving the Homewood community, Baptist Health Brookwood Hospital is a 595-bed acute-care hospital located in Birmingham, Alabama. The facility offers a full complement of services, including emergency care, cardiovascular care, bariatrics, orthopedics and rehabilitation, and surgical services. It also is one of the largest providers of psychiatric care in the state. The hospital has earned Primary Stroke Center certification from The Joint Commission, and its bariatric program has been accredited as a Comprehensive Center by the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP).

The Women's Medical Center provides comprehensive women's services through all stages of life, including gynecologic surgery, maternity care, and labor and delivery services. Additionally, the Level III Neonatal Intensive Care Unit (NICU) holds Advanced Certification in Perinatal Care from The Joint Commission.

Responsibilities

Essential Functions Develops preliminary and comprehensive assessments, written plans of care, discharge plans and progress notes and reviews and revises these plans as necessary. Provides resident, family and/or resident representative with information concerning resident's rights and advance directives. Develops an effective working relationship with the resident, family, team members, physicians, community and others to collaborate, advocate and problem solve on behalf of the resident to allow him/her to attain or maintain his or her highest practicable physical, mental, and psychosocial well-being.

Ensures residents have access to prescriptions, durable medical equipment (DME), and other services as identified upon discharge. Refers residents to education regarding the healthcare delivery and reimbursement systems, prescription drug programs, health & wellness programs, community agencies, public and private organizations, housing options, and other services, as appropriate. Participates in committees and Performance Improvement Projects. Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.

Maintains compliance with all Orlando Health policies and procedures.

Other Related Functions Possesses excellent analytical and team building skills, as well as the ability to prioritize and work independently. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served though knowledge of the principles of growth and development over the life span. Demonstrates awareness of medical/ legal issues, patient rights and compliance with standards of regulatory and accrediting agencies

Qualifications

Education/Training
Master's Degree in Social Work

Licensure/Certification
LCSW or LICSW in the state of Alabama

Experience
One (1) year supervised social work experience in a health care setting working directly with individuals.
