Capstone of Hughes Springs is seeking a full-time Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our home health team. You will provide skilled therapy in patients' homes, evaluate progress, and adjust plans of care to maximize independence and safety.
Collaboration with the supervising PT and other care professionals is essential, along with documenting care accurately and timely in the home health setting.
