Gentiva seeks a compassionate Hospice Licensed Practical Nurse (LPN) to join our team on a PRN basis. You will deliver hands-on hospice care in patients' homes, assisted living, or facilities, reporting to the Administrator or Executive Director.
You will collaborate with an interdisciplinary team, perform patient assessments, and contribute to individualized care plans while providing empathetic support to families during difficult times.
