Contract Length: 20 weeks

Productivity expectations: 30 points weekly

On Call/Weekend Requirement: NO

PAY PACKAGE:

$2,686/week (Gross)

Hourly Pay: $67.15

Breakdown

W2 - $36/hr

Stipends: $1,246/week

Mileage reimbursement available

Experience:

• Minimum 2 years of experience
• Home Care Home Base (HCHB) preferred.

Certifications & Licenses:

• Physical Therapist license for the state of KY
• BLS (AHA)
