Community Action Partnership has an exciting opportunity for

Community Health Worker

In this role, the Community Health Worker (CHW) will assist the CalAIM Housing Navigation and/or the Enhanced Care Management programs and Family Resource Centers by providing support to participants. The CHW will act as an intermediary between their participants and healthcare and/or social services providers, playing a critical part in the work and dedication of Community Action Partnership of Orange County’s initiatives.

Born out of the War on Poverty more than 50 years ago, Community Action Partnership Orange County (CAP OC) is a trusted resource for Orange County community members who face obstacles such as food insecurity, unemployment, economic turmoil and more. The Community Health Worker will function as an integrated team member by providing support for the Housing Navigation, Enhanced Care Management programs, and Family Resource Centers.
